The Center said she was weaned, but she isn't eating what they sent and has solution poop.
Mayb u will have to tt some other food and see what it like. However it has to be change slowly as puppy have weaker stomach. Is the puppy drinking milk? It mayb due to the milk. Mine have this probably so I stopped giving milk and give some milkntreats instead to ensure it has enough calcium for growing bones. Charcoal pills may control the solution poo but I suggest it's better to treat it from the food. Also, puppy tends to have softer poo although not solution.
You could step to the pet store and get some puppy formula and see if he will eat this. You obligation to really mix the dog food into a mush so the dog can eat it. The loose stool is probably due to the new environment. I would take the dog to the vet as soon as possible and hold it checked health wise to be sure it isn't sick. They can also support you on what you can feed her since she is not eating the food you are providing.
You obligation to get the pup to eat, if the gruel that be suggested does not work, go to your local pet store, buy formula and a nursing bottle and feed it like you would a cute little furry child :)
You can try to fix her up some gruel. Go to the pet store and get a can of dog formula.mix it with canned dog food...or whether you are into the raw diet, mix with some ground meat. The puppy will soon learn to drink.
Answers: I find it hard to believe that an adoption center would give you a puppy that is too young at heart to even be away from it's mother. I think it's time to call a vet.

liquid stools is something serious, particularly if it's constant.. take the puppy to the vet.. it could be worms..
also, ask the vet for suggestions for food.. we started our dog on dry food when he be a puppy, but he didn't eat it.. so we put him on pedigree pouches, but he was getting sick from it.. now w

The puppy could be sick I would take the puppy to the vet. You can also ask the vet for any suggestions!!
I'm Guessing the food is not agreeing with her stomach! I used to feed my dog eukanuba and she would have runny poop that be very hard to clean up. Change the food and check next to the vet first. Wow, 5 weeks! The minimum of getting a puppy is 8 weeks, maybe that is why it poop is runny, it is so young!

I raised a single puppy from 3 weeks. We bottle feed him liquid esbilac. At this point, I would think your puppy could eat puppy food mush. Get a giant quality puppy food and mix it with water - permit it soak then mash it until it becomes mushy.
Has he have dewormer? You may need to deworm him at 6 weeks and then again at 12 weeks. Don't forget to put out a small dish of water. You may hold to teach him to drink!
Do you have another dog? THIS IS VERY IMPORTANT: if you do not enjoy another dog to teach him the ropes, please research "how to raise a singleton puppy". A lot of singleton pups can grow up to have behavior problems. However, I raise one, as I said, from 3 weeks and he turned out ok. He had food aggression issues, but we worked on it immediately and now he's fine. I can't write everything you will obligation to know here, but there are many web page on socializing a singleton puppy.
